Patient Rights and Information
In accordance with Texas Health and Safety Code Section 181.105, we provide the following instructions to our patients:
How to Request Your Medical Records
Submit a written request to our office. You may use a form provided by us or email your request to Info@movementresilience.com.
Include your full name, date of birth, the dates of service or records requested, and where you would like the records sent.
Sign and date the request. We may require a government-issued ID to verify your identity.
We will respond to your request within 15 business days as required by Texas law. A reasonable fee may apply for copying and mailing.

How to File a Consumer Complaint
You may file a complaint about this practice with the Texas Board of Chiropractic Examiners:
Visit tbce.texas.gov and select "File a Complaint," or
Download the complaint form from the Board's website, or
Call (512) 305-6700 to request a complaint form by mail.
Submit the completed form by mail to the address above or by email as instructed on the Board's website.
You may also file a complaint with the Texas Attorney General's Consumer Protection Division at texasattorneygeneral.gov or by calling (800) 252-8011.
